Sunnybrook Flu Vaccine Drive-Thru Clinic

ATTENTION: Appointments will become available at 8am EST on Tuesday November 2nd

 

Please note that the vaccination clinic may become fully booked.  New appointments may become available if there are some cancellations.

 

Consent for Vaccination: (mandatory) 

The following applies to everyone in the vehicle registering for the drive-thru flu clinic:

I and everyone in the vehicle registered for the drive-thru flu clinic have read the Toronto Public Health 2021-2022 Influenza Vaccine Fact Sheet https://www.toronto.ca/community-people/health-wellness-care/health-programs-advice/immunization/flu-influenza-vaccines/ 

I understand the expected benefits and possible risks and side effects of the vaccine. I understand the possible risks to myself / my child if not vaccinated. I am aware that I have the opportunity to have my questions answered by a health care provider (HCP) at the flu vaccine clinic, or ahead of time by contacting my primary care provider. I authorize the Sunnybrook Drive-Thru associated HCP to administer the influenza vaccine to myself / my child.

We are committed to protecting your privacy and confidentiality of your personal health information. All information is collected in compliance with the Personal Health Information Protection Act (PHIPA).  In order to meet your health care needs, your personal information will be shared among on-site HCPs and your family physician if possible. By registering for this flu vaccine clinic, you are indicating that you have read this statement and provide consent to the collection, use and sharing of your personal information for the purposes listed above. You are aware that you are providing consent voluntarily. We encourage you to discuss any questions or concerns you may have.

 

By continuing with the online appointment booking process, I acknowledge that I have read the attached influenza vaccine fact sheet. I understand the expected benefits and possible risks and side effects of the vaccine. I authorize the Sunnybrook Drive-Thru associated HCP to administer the influenza vaccine to myself / my child.

PLEASE BE AWARE AND CANCEL OR DO NOT BOOK IF YOU OR ANY PASSENGER IN THE VEHICLE HAS ANY OF THE FOLLOWING SYMPTOMS.

PLEASE CALL YOUR DOCTOR'S OFFICE IF YOU HAVE ANY QUESTIONS 

 Fever 

 Cough 

 Difficulty breathing 

 Sore throat 

 Runny nose 

 Loss of taste or smell 

 Nausea, vomiting, diarrhea 

 Difficulty swallowing 

 

Also, do not book an appointment if you have had a positive COVID test or travelled in the last 14 days.

PLEASE BE AWARE:

 

BY APPOINTMENT ONLY – we will not accept drop-ins. All patients getting the vaccine must be registered. If you are a Sunnybrook employee, please bring your badge with you. 

HEALTHCARD/ GOVERNMENT ISSUED ID – please have health cards or IFH papers for all patients in the vehicle receiving a vaccine  

STAY HOME IF SICK - If you or your child has symptoms like fever, cough, congestion, etc. please cancel your appointment. Do not book an appointment if you have had a positive COVID test or travelled in the last 14 days.

  

BRING YOUR OWN SNACKS and DRINKS – we will not be providing these for safety reasons  

MASKS - Everyone in the car over the age of 2 is required to wear a mask.

The vaccine clinic is available for anyone 6 months of age and older:

 

6 MONTHS to 1 YEAR OLD:

The vaccine will be given in the outside of the upper thigh. Patient must arrive wearing only a diaper/underwear with a blanket covering them on the outside of their car seat.  

Children 6 months to less than 9 years of age receiving influenza vaccine for the first time in their life should receive TWO doses of influenza vaccine at least four weeks apart. Please follow up with your primary care provider or another flu vaccine clinic for second doses. 

 

1-64 YEARS OLD:

The vaccine will be given in the upper arm. Patient must wear a sleeveless or short sleeve shirt that allows access to their upper arm.

 

OVER 65 YEARS OLD:

The HIGH DOSE vaccine will be given in the upper arm. Patient must wear a sleeveless or short sleeve shirt that allows access to their upper arm.

SEATING - The patients receiving the vaccine should be seated next to a vehicle door, or switch seats to allow access. We will not be entering the vehicle to give vaccines. 

 

MULTIPLE PEOPLE PER VEHICLE - When scheduling for more than one patient in a vehicle, you will need to register all patients for the same appointment time.  

 

What else to expect:  

 You will be asked to PARK and TURN OFF YOUR CAR at the time of vaccination  

 Staff will be opening your car door to have appropriate access to administer the vaccine.  

 Staff will be wearing masks and face shields. Hands will be sanitized between cars.  

 YOU WILL BE ASKED TO PARK AND WAIT 10-15 MIN TO ENSURE YOU/OTHERS IN CAR DO NOT HAVE A VACCINE REACTION.

 

 

PLEASE NOTE THAT WE ARE ALSO COLLECING FOOD FOR THE TNO FOODBANK IN SUPPORT OF THE THORNCLIFFE PARK AND FLEMINGDON PARK COMMUNITIES. 

We are accepting donations of:

- Cereal

- Peanut Butter

- Soup

- Canned vegetables and fruit

- Cans of diced tomatoes

- Crackers

- Hygiene items (particularly sanitary pads)
