Check Your Eligibility

The Community Health Clinic serves patients who require timely access to a nurse practitioner for urgent, non-life-threatening conditions.

Please answer the following questions to see if you are eligible for a Community Health Clinic appointment.

Is Community Health Centre the right service for you?

An appointment with a Nurse Practitioner may be the right option for you if you meet all the criteria below:

  • You have a medical condition or injury that is non-life-threatening
  • You have a valid Ontario Healthcard (OHIP)
  • The patient is 18 years or older.

If this is an emergency, go to the Emergency Department.

If you have any of the below symptoms, please DO NOT make an appointment and proceed to the nearest Emergency Department:

  • Chest pain or new abdominal pain
  • Seizures
  • Stroke, such as face drooping, arm weakness, speech difficulties, dizziness, and headaches
  • Severe shortness of breath, dizziness/fainting
  • Internal bleeding
  • Major trauma
  • Fever in infants six months and younger
  • Overdose/poisoning
  • Pregnancy-related emergencies
  • Any serious condition that you feel is getting worse
  • Workplace accident

Please note, Oak Valley Health’s health care provider may not be permitted to prescribe certain medication during your virtual visit, including narcotics, controlled substances or other types of controlled medications.

If you are having a medical emergency or your symptoms worsen while you are waiting for a virtual appointment, call 9-1-1 or visit your nearest Emergency Department in person .
